The Denver Public Schools Transition to Independence Program provides a continuum of services to support students, ages 18-21 with disabilities, to support their post-secondary education, competitive integrated employment, and independent living readiness goals towards independence in the community setting. Programming is individualized to support each student, leveraging their personal strengths, preferences and interests to achieve their post-secondary goals. The Transition to Independence Program goal is to support students to be independent in their communities.

Casemanagers, Job Coaches, and Support Providers are all employees of Denver Public Schools.
Community Partners vary based on the needs of students.
